Đi khám chữa bệnh mang theo giấy tờ gì ?

Tôi quê ở Quảng Ngãi,Tôi được cấp phát thẻ BHXH cho người co hoàn cảnh  khó khăn. Nơi đăng ký KCB ban đầu là TTYT Huyện Bình Sơn, Quảng Ngãi. Xin hỏi nếu tôi đang sống ở TPHCM tôi có thể sử dụng thẻ KCB đó đ ược không? Và khi đi khám tôi cần phải mang theo giấy tờ gì? Và được KCB ở các bệnh viện nào?
